Test for urinary tract infection: $5
Finger stick blood glucose ("blood sugar"): $10
NO OTHER LAB TESTS ARE OFFERED AT SIMPLY FAMILY MEDICINE.
PATIENTS NEEDING OTHER LABS WILL BE GIVEN A LAB ORDER TO TAKE TO THE LAB OF THEIR CHOICE.
Fee for ordering and interpreting outside labs: $10 or $20
SIMPLY FAMILY MEDICINE DOES NOT HAVE AN X-RAY MACHINE.
PATIENTS NEEDING X-RAYS WILL BE GIVEN AN X-RAY ORDER TO TAKE TO THE FACILITY OF THEIR CHOICE.
Fee for ordering basic x-rays: $20
Fee for scheduling major studies (CT, MRI, etc...): $25
Fee for referral to a specialist: $25
Intramuscular Injections:
Flu shot (when available) - for ages 3 and older: $20
Corticosteroid injection ("cortisone shot") - numerous indications: $60
Promethazine ( generic of Phenergan) - for nausea: $25
ALL FEE LEVELS WILL BE REASSESSED, AND POSSIBLY ADJUSTED, ANNUALLY.
